I, the undersigned, hereby acknowledge that I am voluntarily enrolling in and participating in classes, training, and related activities at Tupelo Jiu-Jitsu Academy, located at 811B Varsity Drive, Tupelo, MS 38801.
I understand that Brazilian Jiu-Jitsu is a physically demanding martial art that involves close contact, strength, endurance, and flexibility. I recognize that participation may involve the risk of injury, including but not limited to bruises, strains, sprains, fractures, or other physical harm. I accept full responsibility for these risks and choose to participate voluntarily.
In consideration of being allowed to participate in activities at Tupelo Jiu-Jitsu Academy, I hereby release, discharge, and hold harmless Tupelo Jiu-Jitsu Academy, its owners, instructors, staff, volunteers, and affiliates from any and all claims, demands, actions, or causes of action that may arise from my participation, except in cases of gross negligence or intentional misconduct.
I certify that I am in good physical condition and have no medical condition, injury, or impairment that would prevent safe participation in Jiu-Jitsu. I agree to immediately inform the academy of any changes in my health or physical condition.
In the event of an emergency, I authorize Tupelo Jiu-Jitsu Academy staff to seek medical attention on my behalf if I am unable to do so, and I assume full financial responsibility for any resulting expenses.
I grant Tupelo Jiu-Jitsu Academy permission to use photographs, videos, or likenesses of me for promotional, educational, or informational purposes, including use on social media, the academy’s website, or printed materials, without compensation.
I agree to conduct myself with respect toward all instructors, staff, and fellow students. I will follow the rules, regulations, and safety guidelines established by Tupelo Jiu-Jitsu Academy. I understand that failure to do so may result in suspension or termination of my membership.
As a member of Tupelo Jiu-Jitsu Academy, I understand that consistent attendance and commitment are essential to my progress and the operation of the academy. However, if I choose to cancel my membership, I agree to the following terms:
1. Written Notice Requirement
A written notice of cancellation must be submitted to Tupelo Jiu-Jitsu Academy to initiate the cancellation process. The notice must include my name, membership details, and desired effective cancellation date.
2. 30-Day Notice Period
All cancellations require a 30-day notice period. My membership will remain active for 30 days after the academy receives my written notice, and I am responsible for any dues incurred during this period.
3. Delivery Options
Cancellation notices may be submitted using one of the following methods:
In Person: Submit a written notice at 811B Varsity Drive, Tupelo, MS 38801.
Mail: Send a signed letter with all details to the address above.
Email: Send an email to [email protected] from the email address on file.
4. Responsibility During Notice Period
I understand that I am responsible for dues and fees during the 30-day notice period. No refunds will be issued for dues paid before the cancellation date unless otherwise stated by the academy’s refund policy.
By signing below, I acknowledge that I have read, understood, and agree to this membership cancellation policy.
This agreement shall be governed by and construed in accordance with the laws of the State of Mississippi, and any legal action related to this waiver shall be brought in the courts of Lee County, Mississippi.
I have carefully read and fully understand this Adult Program Waiver and Membership Cancellation Policy. I voluntarily sign it with full knowledge of its contents and implications.
